Individual Registration

If you are traveling with a companion, you will need to fill out a separate registration form. Please fill out this form for an adult or this form for a minor.

Registration Fees

 **Travel Companion Registration: Space for Travel Companions is limited and registrations will be accepted on a first-come, first-served basis. The Travel Companion rate is intended for individuals accompanying a registered conference attendee who will not derive professional benefit from participating in conference sessions or presentations. Travel Companions do not need to hold a CMS Membership.

• Traveling with Minors: Parents or legal guardians must complete the registration form on behalf of minors. No registration fee is required for children younger than six years old; however, attendees traveling with children under six must notify the CMS Conference Planner in advance. Parents and legal guardians are responsible for reviewing and complying with all policies regarding travel with minors outlined in the Terms & Conditions.
